01Scheduling an Appointment

New patients

Please call the office at (646) 926-7495 to schedule. For a faster option, new patients can text their request along with their insurance cards and photo ID — our staff will get back to you to confirm the appointment.

Existing patients

Call the office, or request an appointment directly through the CCWH Patient Portal.

Telehealth

Telehealth appointments are available — just call and request telehealth to get set up.

Please note: there is a $60 fee added to patient profiles for no-call / no-show appointments.

02Insurance We Accept

We are currently participating in-network with:

  • Humana (Medicare)
  • Medicare of New Jersey

Please call to confirm whether we accept your specific plan. Our staff is trained to handle insurance claims, so you don't have to. We also help patients without health coverage and with high-deductible plans find a workable solution, and we accept some out-of-network providers.

03What to Bring
  • Your insurance identification card(s) — a photocopy will be placed in your billing file.
  • If you're covered by more than one insurance, let us know which is primary.
  • Recent lab results, if any.
  • Ultrasound reports and images (usually on a CD).
  • Any relevant medical records.

Payment

Copayments and outstanding balances are expected at the time of your visit. We accept Visa, MasterCard, cash, and personal checks printed with your name, address, and phone number.

04Patient Portal

Save time during your appointment by checking in through your Patient Portal. Patients can access medical records, schedule appointments, and send messages to the practice through the portal.

First-time registration

Once your appointment is scheduled, you'll receive a confirmation email with registration instructions — including a username and temporary password. Please complete your registration and check in via the portal before your appointment.
